Structure: Full-Time | Direct Placement | MondayFriday | Daytime Schedule

About the Opportunity

A rapidly growing mobile healthcare provider is seeking a Wound Care Nurse Practitioner (NP) to join its mission-driven team delivering high-quality, evidence-based care in the Greater Cleveland area. Youll provide direct wound care and primary care services to patients across multiple settings nursing facilities, outpatient clinic and mobile clinic.

This is a highly autonomous role with the ability to build your own schedule with a short runway to a leadership role. What Youll Do

  • Deliver wound care and general primary care services to adult and geriatric patients
  • Conduct assessments, manage acute/chronic conditions, and create individualized treatment plans
  • Perform advanced wound care procedures including debridement, dressing changes, and infection management
  • Educate patients and caregivers on wound prevention and health maintenance
  • Collaborate with interdisciplinary teams to ensure coordinated care
  • Maintain timely and accurate documentation
  • Shadow and train closely with a peer provider before practicing independently

What Youll Need

  • Graduate of an accredited Nurse Practitioner program (MSN or DNP)
  • Active OH RN license and NP certification
  • National board certification (AANP or ANCC)
  • Active DEA license + BLS/CPR certification
  • Minimum 2 years of NP experience in primary care, geriatrics, or independent practice
  • Valid drivers license and reliable transportation
  • Wound care certification (WOCNB or ABWM) is a plus employer will assist in certification if not held, would like to have complete in the first 6 months
  • Strong communication skills and a passion for community-based care
