About the Opportunity

OnDeck Solutions is partnering with a skilled nursing facility in Lebanon, OH, to fill an immediate need for a COTA. Key Responsibilities

• Provide skilled occupational therapy services to residents per the plan of care established by the supervising OT
• Complete timely, accurate documentation in compliance with PDPM, MDS, and payer requirements
• Collaborate with the interdisciplinary care team (OT) to support resident goals
• Monitor and report resident progress, adjusting treatment approaches as directed
• Maintain productivity standards consistent with facility expectations
• Ensure all care is delivered in accordance with state practice act and facility policy

Qualifications

• Active, unencumbered COTA license in OH
• Current CPR/BLS certification
• Prior SNF or geriatric experience strongly preferred
• Working knowledge of PDPM and Medicare Part A/B documentation standards
• Reliable transportation to facility location
• Ability to pass background check and drug screening
